What you'll do

• Identify patients appropriate for Comprehensive Care Clinic services and engage them in care
• monitor data from implanted remote-monitoring devices and coordinate with providers
• document care in the electronic medical record
• communicate with patients and families by phone or virtual visits
• assess patient and family needs
• coordinate referrals and care transitions
• liaise between patients and providers
• provide education and clinical support
• assist with practice guidelines and performance improvement.
