What you'll do

• Provide case management and life-skills services
• help clients access community resources and develop and implement service plans
• follow up on services and progress
• assist with daily living activities and personal goals
• provide transportation to appointments
• plan weekly activities
• respond to incidents according to agency procedures
• support community integration
• document services
• monitor client safety
• facilitate shelter and housing transitions
• develop housing and income goals
• coordinate with agencies and professionals
• help clients apply for and maintain benefits
• conduct outreach
• and complete required reviews and housing checklists.
