What you'll do

• Complete initial nutritional assessments and develop and educate patients on nutrition care plans
• formulate and implement policies for patient and clinic nutrition processes
• advise patients, management, and physicians on nutrition
• monitor monthly lab results and recommend nutritional changes
• participate in short- and long-term multidisciplinary care planning
• develop patient and staff education tools
• maintain required nutritional outcome documentation.
