What you'll do

• Develop treatment plans based on needs assessments, interests, and therapy objectives
• observe and record participation and progress and modify programs as needed
• conduct therapy sessions and encourage leisure activities
• plan and oversee group, small-group, and one-to-one events
• administer interest surveys, MDS, care plans, and progress notes
• attend care plan meetings
• coordinate with health services departments
• evaluate programming
• arrange transportation for off-campus events
• coordinate volunteer screening and training
• administer the Memory Neighborhood activities budget
• report changes in resident condition
• help publish Memory Support newsletters and calendars.

Why this role
