Job overview
The Transition Nurse Liaison (Registered Nurse) is an onsite, full-time temporary assignment serving western Minneapolis suburbs, including Eden Prairie, Minnetonka, Hopkins, Mound, and Chanhassen, MN. The schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., plus a weekend rotation every sixth or seventh weekend. Pay is up to $74.20 per hour, depending on experience, plus mileage reimbursement. The assignment is expected to run from November 23 through March 2027. The nurse coordinates safe, timely transitions from hospital care to appropriate post-acute services. By evaluating needs, coordinating referrals and resources, educating patients and caregivers, and ensuring receiving teams have necessary information and supplies, the liaison supports safe discharge and continuity of care.

Why this role
Requires at least two years of RN work experience, a current Minnesota Registered Nursing License, current BLS certification through the American Heart Association or Red Cross, home care or hospice experience, and OASIS assessment experience. BSN and hospital-based case management experience are preferred.
