What you'll do

• Assist with pre-admission screening
• conduct admission conferences with residents and families
• monitor resident conditions and documentation
• complete PPS and comprehensive assessments within required intervals
• coordinate care-plan meetings and discharge planning
• develop care plans with resident and family input
• track Part A and B case mix weekly
• maintain census and RUG classifications
• coordinate with rehabilitation on classification and resource use
• conduct weekly rehabilitation meetings
• transmit assessments within regulated time frames
• protect confidential information and report incidents promptly.
