What you'll do

• Accept and treat patients within the designated geographic area
• evaluate home-based therapy needs and develop individualized treatment plans
• consult with physicians, patients, and families on goals
• implement therapy programs
• instruct caregivers in therapy techniques
• document visits
• communicate with the care team, including around discharge planning
• and follow agency policies and professional standards.
