What you'll do

• Provide culturally competent patient education and navigation
• identify social determinants of health
• advocate for timely care
• conduct follow-up by home visits, community meetings, or phone
• screen and determine Sliding Fee Scale eligibility and discounts
• assist with Medicaid applications and renewals
• determine Presumptive Eligibility
• document in EHR and eligibility systems
• collaborate with care teams and community providers
• refer patients to community resources
• participate in outreach and enrollment activities.
