Develop and deliver a personalized plan of care for your patients. Uses digital platform to deliver telehealth visits including evaluation, assessment, and treatment plan development. Performs Telehealth and Remote Therapeutic Monitoring (RTM) services. Communicates with the primary treating clinician providing revisions in treatment as appropriate.
