Memory Care Director

What you'll do

• Educate residents and families and address concerns
• collaborate with nursing, therapy, social services, admissions, and dietary teams
• assist with admission and discharge planning
• facilitate communication with physicians and care teams
• conduct satisfaction rounds
• support care conferences and interdisciplinary meetings
• maintain documentation and regulatory compliance
• assist with resident engagement initiatives.
