What you'll do

• Carry out goals in Person-Centered Individualized Support plans
• assist with activities of daily living
• help plan day-program activities
• follow individual high-risk, diet, supplement, and medication protocols
• maintain client safety and confidentiality
• collect data and complete billing and other paperwork
• communicate developments to waiver services leadership
• collaborate with families and treatment teams
• attend meetings and training.
