Case Manager — Runaway and Homeless Youth Program

What you'll do

• Provide case management and targeted case management, including service delivery, documentation, referrals, and reporting
• complete assessments and social histories
• develop and implement individualized case plans
• conduct street outreach and RHY screening to locate and engage runaway, homeless, and at-risk youth
• maintain collaborative relationships with community agencies
• document case activity within five business days
• enter youth data in the RHY-HMIS system
• attend staff meetings and required trainings
• assist with shelter activities and program reports.
