Care Navigator

Job overview

The Care Navigator role is based in Costa Mesa, CA, with work split between in-person visits at long-term care facilities and patients’ homes and office-based phone calls. Compensation is not specified. CareConnectMD provides personalized care for frail and medically complex patients and coordinates care transitions. The navigator engages patients and caregivers, gathers clinical and psychosocial information, and serves as a point of contact for the CareConnect Concierge program. The role supports the interdisciplinary clinical team by sharing patient insights and helping obtain records, results, reports, and status updates.

What you'll do

• Engage patients and caregivers in person and by phone
• build relationships and explain the CareConnect Program
• conduct structured surveys to collect clinical and psychosocial data
• report to and collaborate with the clinical team
• contribute insights to weekly interdisciplinary discussions
• help obtain medical records, laboratory results, imaging reports, and patient status updates
• spend 40% of time on home and long-term-care visits and 60% in the office on phone calls.
