Home Health Registered Nurse (RN)

The RN assesses patients, provides ordered care, and coordinates with physicians and the home care team to help patients maintain independence and ensure care plans and documentation remain current.

What you'll do

• Visit home health patients
• administer at-home IVs and wound care
• assess patients and document findings
• obtain physician orders
• initiate and revise care plans with the Clinical Team Manager
• coordinate with physicians and care team members
• complete recertifications, transfers, and discharges as requested
• conduct Home Health Aide supervisory visits.

Why this role

Availability requested for at least two to three days each week (or ten productivity units), one weekend per month, and two holidays per year. Current California RN license, BLS, nursing license insurance, and IV/phlebotomy certification are required.
