Case Manager, Home Care Management

What you'll do

• Conduct home visits to assess older adults and determine eligibility for state home care services or community assistance
• develop care plans using EOEA guidelines and ensure interdisciplinary review
• implement plans using community resources
• monitor clients and plans through visits, calls, and vendor communication
• document significant communications
• advocate for clients
• consult with managers and staff
• participate in team activities and outreach.
