What you'll do

• Own and grow relationships with facilities, home health agencies, hospice, and other referral sources
• represent Active Vital Care as the primary field contact
• drive patient referrals and registrations
• conduct regular in-person partner visits
• identify and onboard referral partners
• track outreach, referrals, and pipeline activity in CRM
• coordinate with scheduling, clinical, and operations teams
• resolve issues and maintain partner satisfaction
• increase referral volume per account.

Why this role
