What you'll do

• Assess patients for appropriate acute-care admission and level of care
• identify post-hospital needs and arrange ancillary services
• coordinate transfers
• refer patients to allied health personnel and community resources
• consult on complex cases
• communicate with physicians about discharge plans
• advise staff and outside agencies on insurance criteria and regulations
• deliver denial and referral notices
• and maintain patient confidentiality.
