What you'll do
- Conduct admission assessments
- evaluate hospice eligibility and clinical needs
- educate patients and families about hospice services and expectations

- coordinate with physicians, facilities, referral sources, and hospice colleagues
- complete admission documentation
- develop initial care plans
- provide education on symptom management, medications, equipment, and end-of-life care
- coordinate supplies and equipment
