Case Manager

What you'll do

• Visit each foster home on the caseload at least monthly and have face-to-face contact with each child at least twice monthly
• maintain case management documentation, treatment plans, and service plans
• provide guidance, assistance, support, and supervision to foster families
• ensure families are current on training and provide training if necessary
• coordinate child placements, emergency placements, and discharges
• develop and review treatment plans
• attend treatment plan reviews, ARD meetings, court hearings, and staff meetings
• facilitate communication and reporting during serious incidents
• keep agency administration informed
• serve as nights and weekend on-call staff.
