Hospice Registered Nurse (RN) Case Manager — Day

What you'll do

• Manage assigned hospice patients from admission through discharge
• assess and monitor patient status
• develop and maintain individualized plans of care with physicians and the interdisciplinary team
• provide nursing care, symptom management, pain control, and crisis intervention
• educate patients, families, and caregivers
• coordinate weekly team updates and case conferences
• monitor labs, medications, and condition changes
• supervise LPNs, hospice aides, and other clinicians
• document care in the EMR
• participate in the on-call rotation.
