Resident Care Coordinator — Avamere at Chestnut Lane

What you'll do

• Coordinate resident care needs and staffing
• maintain schedules, physician appointment and pharmacy logs, and clinical records
• assist with training new care staff
• support activities of daily living
• conduct audits and assigned resident evaluations
• manage caregiver and medication technician schedules
• assist with resident assessments, move-ins, and transitions
• support caregivers and help ensure supplies and equipment are available
• take on after-hours and weekend on-call staffing responsibilities as needed.
