What you'll do

• Provide clinical services to referred patients in school and/or clinic settings
• consult with schools and agencies
• provide treatment and care coordination
• conduct clinical assessments
• develop integrated wellness plans
• facilitate behavioral and crisis interventions
• coordinate referrals and discharge plans
• provide patient education and behavioral change plans
• maintain medical records and progress notes
• participate in health improvement activities, chart reviews, and assigned meetings.

Why this role
