What you'll do

• Conduct virtual visits for acute, chronic, preventive, and mental health needs
• build patient relationships and continuity of care
• collaborate with virtual care teams, health coaches, behavioral health specialists, and consulting physicians
• participate in weekly Clinical Rounds
• supervise one or more NP or PA colleagues
• refine virtual panel-building workflows
• build and maintain a sustainable patient panel
• meet schedule utilization benchmarks.
