The Registered Nurse (RN), Weekend Home Health Admissions role is an onsite field position serving the Twin Cities Metro Area, based in Saint Paul, MN. Compensation is $200 per Start of Care visit plus mileage reimbursement. The role is weekends only. Home Health Care, Inc. is a locally owned, Medicare-certified home health agency serving Minnesota families. The nurse completes Start of Care admissions and establishes the clinical foundation for each home health episode. Through comprehensive assessment, OASIS documentation, care planning, patient and caregiver education, and coordination with the interdisciplinary team, the role helps patients begin appropriate care at home.

What you'll do

• Complete weekend Start of Care admissions in patients’ homes
• perform comprehensive nursing assessments and OASIS documentation
• develop and initiate individualized care plans
• identify clinical, medication, safety, and disease-management needs
• educate patients and caregivers
• coordinate services with the interdisciplinary team
• complete timely electronic documentation.
