What you'll do

• Assess patients and families and determine hospice needs
• provide nursing care using the nursing process
• develop, initiate, and revise plans of care
• administer prescribed medications and treatments
• counsel and instruct patients and families
• coordinate assigned caseloads when assigned as case manager
• maintain clinical notes
• communicate changes and needs to physicians
• coordinate with community health professionals and the interdisciplinary group
• participate in on-call duties.
