What you'll do

• Conduct client admissions, obtain histories, perform physical assessments, and evaluate home-care needs.
• Implement nursing care plans, provide skilled nursing treatments, document clinical notes and OASIS paperwork when indicated, teach and supervise clients and caregivers, supervise home health aide care, coordinate with providers and other health professionals, and participate in case conferences.
• The posting states Monday through Friday, shifts as needed, and travel to client residences.
