The nurse provides symptom support and responsive care for hospice patients, including phone triage and in-person visits when needs cannot be managed remotely. The work helps patients receive timely assistance while supporting medication refills and durable medical equipment needs.

What you'll do

• Triage phone calls as needed
• complete steps for medication refills
• support and troubleshoot durable medical equipment
• manage hospice patient symptoms
• make visits for deaths or issues that cannot be managed by phone. Active RN licenses in Minnesota and Wisconsin, at least one year of RN experience in hospice, long-term care, or palliative care, and reliable transportation are required.
