• Identify members for care management with providers
• conduct initial and periodic holistic assessments
• prioritize members by need and follow-up
• formulate, implement, and evaluate care plans
• revise plans as needs change
• participate in care conferences
• connect members with community resources
• promote self-management
• conduct transitional follow-up calls after hospital discharge
• coordinate transitions of care and referrals
• maintain timely documentation
• participate in team meetings and peer review
• support orientation of new personnel
• participate in evening and weekend rotation for transitional care needs.
