Lead Case Manager

What you'll do

• Supervise case managers and assign daily work
• oversee case management and housing services
• manage client logs, files, and HMIS reports
• maintain community referrals and housing referral systems
• coordinate with case managers and navigators
• ensure client meetings and case records meet requirements
• advise clients and refer them to resources
• drive clients to appointments and workshops
• develop housing workshops
• track client data and submit reports.
