What you'll do

• Assess patients’ psychosocial status in relation to illness and environment and communicate findings to the registered nurse
• conduct social evaluations and plan person-centered interventions
• maintain clinical documentation
• coordinate referrals and provide information about community services
• serve as liaison with community agencies
• collaborate with facility coworkers
• participate in care planning and case conferences as required
• address resident behaviors and participate in discharge planning
• coordinate medication review regimens.
