The Social Service Worker — Intake Services role is onsite in Salt Lake City, Utah. Pay starts at $23 per hour, calculated based on years of licensed working experience. Valley Behavioral Health’s Access Team helps people connect with behavioral health and substance use treatment. The worker completes intake assessments and ASAM evaluations for Adult Outpatient programs, develops and manages personalized care plans, and coordinates services to support continuity of care. The role provides client-centered support while helping individuals connect with appropriate treatment.

What you'll do

• Conduct intake assessments and ASAM evaluations for Adult Outpatient mental health, substance use, and court-ordered programs
• develop, implement, and manage personalized care plans
• document treatment, transitions, and discharge plans
• review care plans
• coordinate services among agencies and providers
• support continuity of care and meet productivity standards.
