What you'll do

• Implement interventions in Person-Centered Plans
• provide individual, group, and family counseling or support
• support relapse prevention, life skills, crisis planning, and crisis response
• document health-related appointments and services
• maintain communication with the Program Director, recipients, families, and treatment teams
• support case management functions if assigned and monitored by the Program Director
• participate in quality assurance activities.
