What you'll do

• Assess patient and family psychosocial, social, emotional, caregiver, safety, and support needs
• provide social work care under the physician’s plan of care
• offer short-term counseling and crisis intervention
• support advance directives, funeral planning, and care transitions
• arrange referrals and connect families with community resources
• provide and update bereavement care
• participate in interdisciplinary and quality care meetings
• maintain timely clinical and administrative records
• collaborate with supervisors and care team members
• travel to other offices as requested.
