The work contributes to coordinated treatment, patient safety, and discharge planning alongside the interdisciplinary care team.

What you'll do

• Perform psychosocial assessments to identify social service needs and plan care
• evaluate patient data and develop and implement care plans
• communicate patient needs to physicians
• participate in interdisciplinary treatment planning
• complete progress notes and discharge plans
• provide individual support and facilitate family meetings
• report relevant changes in mental status, behavior, or treatment response to the care team
• maintain knowledge of community social, medical, and psychiatric resources
• communicate with families and caretakers
• collaborate with the Utilization Review Coordinator and healthcare team
• and support patient safety. SWT or LSW is required. Field hours are flexible
• some evenings and/or weekends may apply.
