The Medical Data Set Coordinator – Registered Nurse role is onsite in Seymour, WI. Pay is $40–$41.75 per hour, based on experience. This is a part-time day-shift position, Monday–Friday, for 30 hours per two-week pay period, supporting resident nursing services and long-term-care assessment and documentation. The coordinator completes resident assessments and required MDS and Medicare documentation, coordinates interdisciplinary care planning, and supports nursing services for residents. The role helps ensure care plans and records reflect resident needs and applicable state and federal requirements.

• Complete MDS and Medicare assessment documentation
• assess resident health and communicate with the care team
• coordinate admissions, discharges, and level-of-care reviews
• coordinate interdisciplinary assessments and care plans
• lead or delegate care conferences
• document resident care
• supervise and direct team members
• administer medications and treatments as ordered
• respond to emergencies
• support staff orientation and development.

Why this role
