Hospice Field Registered Nurse (RN) — Evening/Nights

The Hospice Field Registered Nurse (RN) — Evening/Nights role is an onsite field position based in Englewood, CO, with an annual salary of $75,000–$98,000. The schedule is 5:00 p.m.–8:30 a.m., Monday through Thursday. Choice Hospice provides patient-centered care to people with life-limiting illnesses and support to their families. The nurse assesses patient needs, develops and coordinates care plans, and works with an interdisciplinary team to support comfort and continuity of care. The role also educates patients and families, documents progress, and provides clinical direction to hospice aides and LPN/LVNs.

What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, document, and revise care plans
• educate patients and families
• determine service scope and frequency
• provide clinical direction to hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• perform assigned on-call responsibilities.
