Social Worker — Home Health and Hospice

What you'll do

• Provide supportive counseling aligned with patient and family needs

• help physicians and team members understand social and emotional factors related to patient health
• contribute to patient-centered transition plans
• maintain clinical records
• perform other duties delegated by the Clinical Director or Supervisor.
