What you'll do

• Provide direct case management and service coordination for SOT program participants
• maintain timely case records
• explain benefits and assist with applications
• assess needs and coordinate referrals
• provide interpretation support when appropriate and coordinate interpreters when needed
• build community organization relationships
• participate in supervision and team meetings
• support language access and equitable service delivery.
