The role supports patients’ care management and independence while serving as case manager for assigned patients.

What you'll do

• Provide in-home nursing visits according to physicians’ plans of care
• assess patients’ health, history, and home environment
• develop and update individualized care plans
• administer prescribed medications and treatments
• educate patients and caregivers
• monitor progress and communicate changes
• document visits
• coordinate with therapists, aides, and other team members
• manage assigned cases and support discharge planning.
