What you'll do

• Assign and monitor team caseloads
• review referrals and screenings
• designate clients for intake and verify supporting documents
• provide information and referrals
• conduct client assessments and develop case plans
• coordinate support systems, move-ins, rental negotiations, and referrals
• make home and field visits as needed
• monitor service quality
• review cases with program leadership and the team
• audit case files
• conduct crisis intervention as necessary
• coordinate services for veterans and families
• support case conferencing, counseling, advocacy, workshops, and outreach
• maintain secure client records.
