Registered Nurse (RN) Home Care Case Manager

What you'll do

• Conduct new-patient start-of-care assessments and scheduled or as-needed reassessments
• manage caseloads, care plans, and documentation
• guide field nurses on complex cases
• communicate with physicians, families, and interdisciplinary team members
• ensure regulatory and Joint Commission compliance
• identify and escalate clinical concerns
• participate in office meetings and case conferences.
