What you'll do

• Build relationships with seniors, community organizations, healthcare partners, and local businesses
• conduct outreach at community settings and events
• communicate the organization’s value proposition
• plan and participate in outreach events and presentations
• develop relationships with agents, brokers, social workers, case managers, and senior housing managers
• schedule meetings with prospective patients and referral sources
• use Salesforce CRM to track leads and interactions
• maintain outreach records and activity calendars
• support marketing and re-engagement campaigns
• strengthen referral networks
• support patient retention and address concerns.

Why this role
