What you'll do

• Provide supportive services to individuals with acute behavioral health challenges
• encourage self-care and activities of daily living
• assist with scheduling and keeping appointments
• observe individuals, families, and home environments and share relevant feedback with the treatment team
• encourage cooperation with treatment
• help individuals and families access community resources with the Case Manager and Nurse.
