By coordinating with the interdisciplinary team and educating patients and families, the role supports patient safety, recovery, and discharge planning.

What you'll do

• Perform comprehensive patient assessments and develop individualized plans of care
• administer medications and treatments according to physician orders and unit protocols
• monitor changes in condition and intervene or escalate care as appropriate
• provide wound care and post-surgical monitoring
• assist with activities of daily living
• educate patients and families about disease management, medications, and discharge instructions
• document assessments, interventions, and patient responses in the medical record.

Why this role
