What you'll do

• Participate in initial assessments and service-plan development
• assess clients and document observations at visits
• provide nursing care including wound, catheter, ventilator, IV, diabetic, ostomy, tracheostomy, feeding, post-operative, and medication-related care
• coordinate with physicians and therapists
• educate patients and families
• reevaluate nursing needs
• take verbal orders
• make referrals
• train staff
• supervise CNAs, PCAs, and nurses
• investigate incidents and communicate schedule or care changes.
