What you'll do

• Coordinate access to specialty care and services
• assess patients and develop case management and referral plans
• educate patients and families using evidence-based, culturally relevant materials
• arrange appointments and transportation
• follow up on attendance, reports, and discharge plans
• communicate results and recommendations to primary care teams
• connect patients with community resources
• conduct chart reviews and clinic follow-ups
• participate in care conferences and community meetings
• document education and care activities while maintaining confidentiality and HIPAA requirements.
