What you'll do

• Complete and submit timely, accurate MDS assessments
• select appropriate ARDs and diagnosis codes for PDPM classification and reimbursement
• monitor PDPM components and validate supporting documentation
• ensure CMS compliance
• coordinate care planning across Nursing, Therapy, Social Services, Dietary, and Activities
• lead and participate in interdisciplinary meetings and care-plan conferences
• use listed assessment and documentation systems.
