Associate Care Coordinator

The Associate Care Coordinator is an onsite role in New Orleans, LA, with normal business hours Monday through Friday, 8:00 a.m.–5:00 p.m. Compensation is not specified. The role supports Sleep Management’s patient care coordinators and sales teams in territory growth and referral relationships. The coordinator helps move referrals and service-related work forward while connecting clinical and sales teams with physicians, hospitals, pulmonary practices, case managers, and other referral sources. The role also supports ongoing durable medical equipment coverage and territory business development.

What you'll do

• Coordinate prompt and accurate processing of referrals, documentation, and service activities
• support patient care coordinators and sales teams
• build and maintain referral-source relationships
• conduct routine referral-source visits
• obtain re-authorization documentation for ongoing durable medical equipment coverage
• share educational materials and company updates
• maintain CRM and territory records
• assist with lead generation, pipeline support, account management, and territory planning
• and provide territory coverage during PTO, training, travel, and other business needs.
