The Case Manager, Child & Adolescent Intensive Care Coordination role is onsite in Hampton, VA, with an annual salary of $53,569. The schedule is Monday–Friday, 8:30 a.m.–5:00 p.m., with some on-call responsibilities. Hampton-Newport News Community Services Board’s case management services link families, children, and the supports they need. The role provides intensive coordination for children and individuals with serious emotional disturbance or intellectual or developmental disabilities. Through assessments, individualized service planning, referrals, progress tracking, advocacy, and crisis response, the case manager helps align timely and appropriate services across schools, providers, and community supports.
