What you'll do

• Provide care to residents, including post-operative care, wound management, and chronic disease management
• conduct initial and ongoing assessments of physical, emotional, and cognitive needs
• develop and implement individualized care plans with the interdisciplinary team
• administer prescribed medications, treatments, and therapies
• monitor and report progress to the nursing team and physicians
• collaborate with healthcare professionals
• educate and support residents and families.
